4th Street Clinic Policies
Clinic Policies
Prescription Refills
Please plan ahead and prepare for your prescription refill before you are out of medication. Check before all appointments to see if you are due refills and discuss this with your physician.
To refill a prescription please book an appointment. Your physician will prescribe enough medication to last you until your next scheduled review. Under exceptional circumstances we will accept faxed prescriptions, however if you are requesting a refill by fax only for convenience please be aware this is an uninsured service (not covered by Alberta Health Care). This is therefore a service billable to the patient and will result in a fee of $40.
If you are completely out of medication and contact with the clinic is not possible, most pharmacists will provide enough medication to last until you are next able to see your physician.
We are not able to refill prescriptions over the phone.
Cancellation Policy
Patients wishing to cancel or change appointments must call the office or leave a voicemail message 24 hours before their appointments. Alternatively they may do so on the healthmyself portal. Emails for appointment requests or cancellations will not be accepted.
It is not possible to accept e-mails from patients as this is not viewed by the Alberta OIPC as a secure method of communication.
Failure to notify the office of a cancellation will be deemed a “no show” (see our no show policy).
No Show Policy
Patients who miss their appointments and fail to cancel at least 24 hours in advance may be charged a $40.00 fee for a routine visit or $100.00 for a comprehensive visit. Please call 403.640.4320 to cancel an appointment. If leaving a voicemail to cancel an appointment, then please include your name, physician, appointment date and time.
Alternatively, if you are already registered you can cancel or reschedule using Helathmyself.
Results
Our clinic reception and nursing staff are not able to provide results over the phone. This is to ensure security and privacy of patient medical information. Should you want to review results please book an appointment with your physician.
Patients can now sign up for MyHealth Records at www.myhealth.alberta.ca/myhealthrecords This will allow you to securely check some common lab results, past immunizations and medication profiles.
Email Policy
The 4th Street Clinic is unable to send results, reports, or other documents containing personal health information by email as it is not viewed by Alberta OIPC as a secure method of communication. The clinic will provide their email if required on a case by case basis.
